HIPAA Notice of Privacy Practices

Our commitment

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Padgett Medical Center is required by law to maintain the privacy of your protected health information (PHI), to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

How we may use and disclose your PHI

We use and disclose your PHI for treatment (coordinating your care with the providers involved), payment (billing and collecting payment from you or your insurer), and healthcare operations (quality review, staff training, and internal audits). We may also use or disclose PHI when required by law. We do not use or disclose your PHI for marketing without your written authorization, and we never sell your PHI.

Your rights

You have the right to access and request a copy of your records, request corrections, request restrictions on certain uses and disclosures, request confidential communications, receive an accounting of certain disclosures, and obtain a paper copy of this notice. You may also file a complaint with us or with the U.S. Department of Health and Human Services if you believe your privacy rights have been violated. We will not retaliate against you for filing a complaint.

Changes to this notice

We reserve the right to change this notice and to make the revised notice effective for PHI we already have as well as any information we receive in the future. The current notice will be posted here with its effective date.
