Medical Records Policy

This policy explains how to access your medical records at Padgett Medical Center Ocala and outlines our related office policies. It works alongside our Privacy Policy and HIPAA Notice.

Requesting your records

You have the right to access and receive a copy of your medical records. To request your records, contact our office and we will provide the appropriate authorization form. You may request your records in paper or electronic format, and you may ask that a copy be sent to another provider you designate.

Authorization and turnaround

For your protection, we require a signed authorization before releasing records, especially to a third party. We respond to records requests within the timeframe required by applicable law. Reasonable, legally permitted fees may apply for copies; we will let you know in advance.

Corrections and restrictions

If you believe information in your record is incomplete or inaccurate, you may request a correction. You may also request restrictions on how your information is used or disclosed. We will review and respond to your request as required by law.

Appointments and cancellations

We ask for at least 24 hours' notice to change or cancel an appointment. Cancellations with less than 24 hours' notice, no-shows, or arrivals more than 15 minutes late may be subject to a $25 fee. This helps us keep appointment times available for everyone.
